American Dental Laboratory Service Agreement

By signing the ADL (hereafter referred to as the Company) prescription form, the licensed dentist (hereafter referred to as the Customer) agrees to the
terms stated below.

CASE SPECIFICATIONS:

The Customer shall supply the Company in writing with all specifications and information reasonably required by the Company to prepare the prosthesis requested by the Customer. The Company relies upon the accuracy of information and specifications provided by the Customer and may relyentirely upon the original specifications and information supplied by the Customer without any duty of investigation.

Any restoration that fails due to a defect in workmanship will be replaced at no charge. Please provide the date that the case was originally seated and the details of why the restoration failed. Original restoration and original model work must be returned with the case to receive any credit or warranty
work.

Warranty: American Dental Laboratory provides a 5-year warranty on fixed restorations and a 1-year warranty on removable restorations against defects in materials and workmanship.

PAYMENT: This case shall be paid for in full at the next statement of billing, no later than the 15th of the month. If payment is late, 2% each month interest rates will apply and will be included on the next statement.

In consideration of credit being extended by the Company to the aforesigned licensed dentist(s) for goods and services to be purchased, whether dental practitioner be an individual, proprietorship, partnership, corporation, or any other entity, the signed guarantor(s) each hereby contract and guarantee to the Company the faithful payment, when due, of all accounts for purchases made within the time specified on invoices and/or statements.

Dentist agrees to pay any collection costs incurred to collect the amount owed, including reasonable attorney’s fees. Any revocation of this guarantee shall be in writing and delivered to:

TURNAROUND TIME AND RUSH CASES:

Please schedule ten working days for all cases not including day of pick-up and delivery. Rush cases must be approved prior to scheduling.

Rush Fees: • 6–8 Day Rush: +10% of case total • 3–5 Day Rush: +50% of case total, 15 days on implant cases. Call for turnaround times on removables.
